This pediatric screening test was designed by the West Virginia Vision Initiative for Children for quick amblyopia screening. This pass/fail wall chart contains two rows of LEA SYMBOLS®, one for each eye of critical line 10/20 for 3 year olds and 10/16 for 4 year olds and older. Includes a response key. Instructions are printed on the reverse side. Has a hole for hanging the chart on the wall.

Testing distance: 10 feet/3 meters.
